4^th Grade Behavior Plan

*Students will start each day on Green.

*1^st offense – verbal warning

*2^nd offense – turn to yellow , in class time out (10 minutes)

*3^rd offense – turn to blue, out of class time out with a discipline report, phone call home (the discipline report must be completed in 45 minutes or the student will move to the next step)

*4^th offense – turn to red, Referral will be written
